Users could not reach some internal numbers from certain locations on campus at UNSW.

The issue was related to the power outage and this was addressed by UNSW Facilities Management.

Impact

On Monday, the Lotus Domino server service was restarted multiple times however continued to fail generating NSD files that has been forwarded to IBM for analysis. IBM responded with root cause of the Domino issue - corrupt Full Text Index. This is an issue to which IBM is currently testing a hotfix - but is not available to be installed as yet.

During intermittent outages any mail that was sent to users will have been queued and delivered during server uptimes. Users may experience discrepancies with their Read/Unread marks due to the manual failover process.

Lotus Domino server has experienced recent outages caused by end-user Full Text Index search on mail files. Software Bug identified by Software Vendor in Lotus Domino. IT at UNSW requests that customers do not perform any full text index searches on their mail file until after the hotfix has been applied to the Lotus Domino mail servers.

Workaround if you cannot access Lotus Notes mailbox:
Start Lotus Notes client
Click on File > Database > Open > Server > Put IP Address 129.94.112.225 >
Open > Force Connection directly to CITI server 1 > Create

Notes

Hotfix for Lotus Domino server full text index search software issue has been successfully installed on both IT at UNSW and Chancellery Domino servers.

Client access, mail routing and Domino web-access has tested successfully.
Index on the file that was identified with the root cause of issue has been re-created and so far has tested successfully.

Server up time at time of communication - 1 hr 15 mins

Optus has replaced telephony terminal equipment at Kensington exchange and has replaced most of the copper pairs between Kensington exchange and main distribution fames in Mains Bldg and the Library.
 
On Wednesday, 25th of June, the Optus Multiline service (Rte 163) has been restored initially for handling incoming calls only (for risk mitigation outgoing calls were routed through the Lower Campus PABX). Currently the service is restored for both incoming and outgoing calls

Corruption in managing display profiles caused the portal to react on abnormal and not standard manner. Code has been changed to refer conditional profiles to a default HTML profile. Mainstream browsers (IE, Mozilla Firefox, Safary, Opera) can access and see content on myUNSW. The issue may be residual for not mainstream browsers.
